🔁 How to Refill Your Weight Loss Medication

To better support your individualized dosing schedule, we’ve updated our refill process. Please follow the steps below based on how you receive your medication:

📦 If your medication is shipped to you from a compounding pharmacy:

• Please send your refill request via text message, QR code or through your patient portal.
• Include your full name, medication name and dose, and date of your last dose, if possible.

🏥 If your medication is filled at a traditional pharmacy:

• Ask your pharmacy to send a refill request directly to our office.
• Alternatively, you may also send a refill request via text message or through the patient portal.

🩺 If you are a 4 Cardinals DPC Member:

• You’re all set! Your medication will be automatically shipped and your invoice will be auto-drafted monthly.
